2021 Individual Actions
Mr. Maurice Omaits (IA-20-054)
On December 15, 2021, the NRC issued a Notice of Violation to Mr. Maurice Omaits, a senior engineering training instructor employed by Entergy Operations, Inc. (licensee) at the Grand Gulf Nuclear Station, for willfully compromising engineering support qualification exams by providing inappropriate assistance to students in violation of 10 CFR 50.5, "Deliberate misconduct." This is a Severity Level III violation in accordance with NRC Enforcement Policy Section 2.2.1.d.
Ms. Lilly Porter (IA-21-043)
On October 7, 2021, the NRC issued a Notice of Violation to Ms. Lilly Porter, formerly a Security Shift Supervisor at Grand Gulf Nuclear Station, for a Severity Level III violation involving 10 CFR 73.56(g)(1) which requires, in part, that any individual who is maintaining unescorted access under 10 CFR 73.56 shall promptly report to the reviewing official, his or her supervisor, or other management personnel designated in site procedures any legal action taken by a law enforcement authority to which the individual has been subject that could result in incarceration or a court order that requires a court appearance, including but not limited to an arrest. Specifically, Ms. Porter failed to report to her supervisor that she was arrested by the Mississippi Highway Patrol on February 2, 2020.
Mr. Richard Ellison (IA-21-035)
On September 30, 2021, the NRC issued a severity level III notice of violation to Mr. Richard Ellison, formerly a Senior Reactor Operator at River Bend Station, for engaging in deliberate misconduct that caused Entergy Operations, Inc., River Bend Station, to be in violation of 10 CFR 73.54(b)(2) which requires, in part, that the licensee establish, implement, and maintain a cyber security program for the protection of the safety, security, and emergency preparedness assets from cyber attacks. Specifically, Mr. Ellison, deliberately provided a critical digital asset key to a maintenance supervisor, even though he knew the supervisor was not authorized to have the key.
