IALETTER 09/18/1998 On September 18, 1998, a letter was issued to the individual documenting the NRC's conclusion that he deliberately violated requirements when he compromised the integrity of an NRC reactor licensing examination. The letter also documented that the individual was convicted of a misdemeanor and that one of the items listed in his Plea Agreement was, ". . . defendant also agrees to never reapply for a position of employment with any facility under the jurisdiction, administration, or control of the NRC . . ."

IAORDER 08/02/2005 On August 2, 2005, an immediately effective Order Prohibiting Involvement in NRC-Licensed Activities (for five years) was issued to the individual based on his deliberate activities involving his possession of byproduct material without an NRC or an Agreement State license. The Order is effective for 5 years from the date of issuance of the Order. As a technician formerly employed by Professional Inspection and Testing Services, Inc., the individual maintained possession of a moisture density gauge without the licensee's knowledge.

10/07/2021 On October 7, 2021, the NRC issued a Notice of Violation to Ms. Lilly Porter, formerly a Security Shift Supervisor at Grand Gulf Nuclear Station, for a Severity Level III violation involving 10 CFR 73.56(g)(1) which requires, in part, that any individual who is maintaining unescorted access under 10 CFR 73.56 shall promptly report to the reviewing official, his or her supervisor, or other management personnel designated in site procedures any legal action taken by a law enforcement authority to which the individual has been subject that could result in incarceration or a court order that requires a court appearance, including but not limited to an arrest.  Specifically, Ms. Porter failed to report to her supervisor that she was arrested by the Mississippi Highway Patrol on February 2, 2020.

09/01/2022 On September 1, 2022, the NRC issued a notice of violation for a Severity Level III violation to Dr. Pyko, an interventional radiologist working at Indiana University-IUPUI/IU Medical Center Campus (licensee) in Indianapolis. Dr. Pyko failed to comply with Title 10 of the Code of Federal Regulations (10 CFR) 30.10(a) and placed the licensee in violation of 10 CFR 20.1502(a) when he deliberately failed to wear his assigned dosimetry and did not allow the licensee to monitor occupational exposure to radiation sources under its control.
