The reported condition involved a defect in environmentally qualified Motor Operated Valves which had been installed at the plant. Subsequent to that initial notification, we have learned of further test data that appears to conclude that the MOVs were operable regardless of the defect. However, since we have not yet fully analyzed this test data and determined that we are in full agreement with the conclusion based on that data, we are conservatively submitting this report under Part 21. The requirements of 10 CFR 21.21 (d)(4) identify specific information to be included in this written notification. That information is provided in the attachment to this letter. Component: Motor Operator Model: SB-00-25; SMB-00-25; SMB-00-15; SMB-00; SMB-000 (iii) Identification of the firm supplying the basic component which contains a defect. Limitorque Corporation, Lynchburg, VA (iv) Nature of the defect and the safety hazard which could be created by such a defects. T-drain not drilled through vertically and would act as a pipe plug. Condensation buildup inside the motor would not be relieved resulting in possible degraded motor performance. BACKGROUND (v) The date on which the information of such defect was obtained. Concerns regarding the quality of the Limitorque T-Drains were raised initially on May 21, 1997. In accordance with Company procedures, a determination that this component may fall in the scope of Part 21 was made on September 15, 1997 and a formal evaluation commenced. The evaluation was completed on November 17, 1997 and communicated by facsimile under Part 21 on that day. (vi) In the case of a basic component which contains a defect, the number and location of all such components in use at the facility subject to the regulations in this part. Seven total, four located in the Primary Auxiliary Building and three located in containment. (vii) The corrective action which has been taken. The improperly drilled T-drains were replaced with correctly drilled T-drains. (viii) Any advice related to the defect that has been given to purchasers or licensees.
