THE INGERSOLL-DRESSER PUMP COMPANY SUBMITTED A PART 21 REPORT ON
A POTENTIAL SAFETY HAZARD FROM THE POTENTIAL BREAKAGE OF THE CAST
IRON SUCTION HEADS ON ONE OF THEIR PUMPS.
THE PUMPS IN QUESTION ARE OF THE MODEL APKD, WHICH WERE
MANUFACTURED BY INGERSOLL-RAND COMPANY IN THE MID 1970's. THESE
PUMPS ARE INSTALLED AT SEVEN DIFFERENT NUCLEAR SITES(SIX DOMESTIC
AND ONE FOREIGN). THE SIX DOMESTIC FACILITIES ARE LIMERICK, HOPE
CREEK, SUSQUEHANNA, HATCH, LASALLE AND WNP2. THE FOREIGN
FACILITY IS THE CHINSHAN FACILITY IN TAIWAN. THIS CONDITION WAS
DISCOVERED AFTER A FAILURE OF THE SUCTION HEAD ON THE "1B" RHR
PUMP HAD OCCURRED AT THE LIMERICK UNIT 1 PLANT. IT WAS INITIALLY
NOTED ON 5/98 WHEN A PIECE OF METAL FOUND INSIDE THE REACTOR
VESSEL WAS TRACED TO THE "1B" RHR PUMP. A VISUAL
INSPECTION(BOROSCOPE) REVEALED THAT A PORTION OF THE SUCTION HEAD
WAS MISSING. THESE PUMPS HAVE BEEN USED IN RHR, CORE SPRAY, LOW
PRESSURE CORE SPRAY AND HIGH PRESSURE CORE SPRAY APPLICATIONS AT
THE VARIOUS SITES WHERE THEY ARE INSTALLED.
