Pursuant to 10CFR Part 21 Paragraph 21.21 (b), Rosemount Nuclear Instruments, Inc. (RNII) is writing to inform you that certain data contained in the qualification and type test reports for Rosemount Model 1154 Series H Alphaline Nuclear Pressure Transmitter, which were used to specify the radiation performance identified in relevant product data sheets and manuals, were incorrectly tabulated and reported. This results in a small change to the 1154 Series H Nuclear Pressure Transmitter 30-minute radiation accuracy specification. All other radiation and other accident level specifications are unaffected.

As documented in Section 7.1 of ref. (a), excerpts attached as Appendix A, a radiation specification was developed for Rosemount Model 1154 Series H Nuclear Pressure Transmitter based upon an envelope of observations of output shift (in mV) in 13 different test transmitters while exposed to radiation testing at an average dose rate of 2.07 Mrads p er hour. The resultant specification, shown on page 4 of ref. (c) and page 4-1 of ref. (d), excerpts of which are shown in Appendices C and D, states that during the first thirty minutes of radiation exposure, output accuracy will be within +/- (0-2% of upper range limit + 0.2% of span). This specification was based on the observation that 13 qualification test units appeared to perform within these limits. However, a review of ref. (b) shows that the test data supporting the 30-minute radiation specification were incorrectly transposed from actual raw test data.
The tabulated data in Table 31 of ref. (b), contained in excerpts attached as Appendix B, compares the output shift observed during the first 30 Minutes of the radiation test with the allowed output shift using the 30- minute specification, Table 28 of ref. (b), also shown in Appendix B, exhibits data collected at various points during radiation testing. The 15- minute output data from Table 29 was incorrectly transposed into Table 31 of ref. (b) as the output shift seen during the first 30 minutes of accident radiation. The original strip charts from the test confirm this transposition. This transposition error also occurs in Table 3 of Section 7.1 of ref, (a), shown in Appendix A.

Upon entering the correct 15 minute and 30 minute data into Table 31 of ref. (b), 5 of the 13 test units do not perform within The 30 minute specification (See corrected revision to Table 31 below), Therefore, the conclusion stated on page 10 of reference (a), "All of the test units performed within this new specification"was incorrect. Also incorrect was the statement "All of the test units performed within the thirty minute specification" on p. 72 of ref (b), shown in Appendix B. The specification of +/- (0.2% of upper range limit + 0.2% of span) did not envelope all of the test observations. Consequently, Rosemount has opened the limits using the corrected output shifts, resulting in a new specification of +/- (0-25% of upper range limit + 0.75% of span) including 15 minute and 30 minute excursions within the first 30 minutes. The results of comparing the corrected test results against the revised interval are exhibited in Table 1 below. The test results for all 13 qualification units fall within the revised specification.

Specified radiation performance of any Model 1154 Series H Nuclear Pressure Transmitter can be determined from the corrected original test data shown in Corrected Table 31 and the revised maximum output shift of +/-(0-25% of upper range limit + 0.75% of span) within the first 30 minutes. All subsequent shipments of the Model 1154 Series H Nuclear Pressure Transmitter will be to this revised specification. Moreover, the qualification test report will be revised within 30 days to reflect the new maximum output shift, Until such time as the Product Data Sheets and the Manuals are revised to reflect the new 30 minute radiation accuracy of +/-(0.25% of upper range limit + 0,75% of span) errata sheets will be issued to show These changes. Additional errata sheets will be made available for users requesting them to include in 1154 Series H manuals already shipped from Rosemount Nuclear Instruments, Inc. Following these actions, we consider this matter closed.

Excerpts from Reference (a) Rosemount Report D8700096, "Qualification Report for Rosemount Model 1154 Series H Pressure Transmitter"

  • +10% margin was added to the acceleration requirements at the mounting point of the equipment (the Test Response Spectrum exceeded the maximum acceleration of the Required Response Spectrum by 10%).
  • Margin was applied to the environmental transient by performing the peak transient (without temperature and pressure margin) twice. IEEE 323-1983 states that margin may be applied in two ways. Either a +15 Degrees F temperature margin and a +10% pressure margin may be added to the peak transient, OR the peak transient without temperature and pressure margin may be applied twice. Rosemount has chosen the second method of applying margin during the environmental transient. Therefore, the LOCA/HELB simulation consisted of a margin transient as well as an accident transient.
  • A one year post-accident aging simulation was conducted to provide a +10% margin on equipment operating time following the start of the DBE. The post-accident aging was conducted to demonstrate operability for up to one year after the DBE.

7.0 DESIGN BASIS EVENT SIMULATION

The Design Basis Event was simulated by subjecting the aged test units to accident tests performed in the following order: Accident Radiation' Test, Seismic Test, and LOCA/HELB Test. The results of this testing is described in the following.

7.1 Accident Radiation

The test units were exposed to an average total integrated dose (TID) of 57.91 Mrads from a Cobalt 60 (gamma) radiation source as described in paragraph 1.0.0 of Reference 2.5. A minimum exposure to 55.5 Mrads was required which included 50 Mrads accident dosage, 0.5 Mrads of background radiation and 10% margin an the accident dosage (5 Mrads).

The following dose rate schedule was achieved: 2.07 Mrad/hr for 2 hours; 1.50 Mrad/hr for 4 hours; 1.20 Mrad/hr for 45.5 hours. The testing was conducted at Process Technology in West Memphis, Arkansas.

Table 2 summarizes the maximum shifts that were seen during the test. It was from this data that a new accident radiation specification of +/-(0.5% URL + 1.0% Span) was developed. All of the test units, with the exception of S/N 402847, performed within this new specification. Test unit S/N 402847 did not exhibit a "hard failure" (where the transmitter does not operate) however, it did perform outside of the acceptance criteria. Because it was necessary to know what was responsible for this performance, the test unit was submitted for failure analysis. See Appendix 4 of Reference 2.5 for the failure analysis report. Table 3 summarizes the maximum shifts that occurred during the first thirty minutes of radiation exposure. A new specification of +/-(0.2% URL + 0.2% Span) will apply for the first thirty minutes of radiation exposure. All of the test units performed within this new specification.

Qualification of the Model 1154 Series H pressure transmitter during accident radiation conditions has been demonstrated by the performance of these test units after previous exposure to thermal and functional aging.

Appendix B

"Type Test Report for Rosemount Model 1154 Series H Pressure Transmitter" particular reading could be taken. Table 29 presents data from the accuracy checks that were made periodically throughout the test.

Table 30 shows the maximum shifts that were seen during the test. It was from this data that a new accident radiation specification of +/-(0.5% URL + 1.0% Span) to 55 Mrads was developed. Table 31 shows the maximum shifts that occurred during the first thirty minutes of radiation exposure. In the past, Rosemount has never specified the performance during the first thirty minutes. since there are many customers interested in this information, a new specification of +/-(0.2% URL + 0.2% Span) will apply for the first thirty minutes of radiation exposure.

All of the test units performed within the thirty minute specification. All of the test units, with the exception of S/N 402847, performed within the 55 Mrad specification. Because of excessive drift in output of S/N 402847 during radiation, it was pulled from the test program upon return to Rosemount and submitted for failure analysis (see QTDR 021). Since the transmitter did not return to Rosemount until after. the seismic testing that followed, this transmitters was allowed to be subjected to the seismic event.

The failure analysis (F.A.R. No. 1150) could not recreate the failure of S/N 4028471 Indicating the transmitter had corrected itself sometime between radiation and failure analysis. Failure analysis was, however, continued (F.A.R. No. 1231) in an attempt to isolate the problem to either the transmitter electronics or the module. Although the previous problem of output drift during thermal aging disappeared following replacement of the electronics, the high grounding shifts that occurred with two different replacement electronics indicates it wasn't a problem with the electronics. Analysis of the circuit boards found them to be working properly, thus the problem was isolated to the module. Further testing of the module did not unfortunately expose any problems. The module performed normally during an accuracy check, capacitance check, and testing of the module with temperature. Based on the transmitter performance during the qualification test events and results of the failure analysis, it is hypothesized there was current leakage in the module electronics resulting in drift and intermittent shorting to ground. Moisture present in the module, due to either residual flux or humidity, could produce these effects when a potential leakage path exists in the module electronics. This potential leakage path would be present if there were cracks in the insulation of the wires or electronic components or if the solder pads were too close together on the circuit board. Because the module corrected itself it is
