Subject: Request for Extension
1. | NLI requests an extension until September 10, 1999 to determine the reportability of the matter under investigation. The rationale for this request is as follows: |
| a. | As described in the referenced document, NLI is investigating the cause of a premature over voltage protection actuation in a circuit specific application. The premature over voltage protection actuation occurred during diesel generator start up. Specifically, the premature over voltage protection actuation occurs during diesel generator loading. |
| b. | We continue to be unable to duplicate the condition in laboratory testing. |
| c. | Our conclusion to date is that the failure is unique the particularities for the circuit in which the power supply is used and is not reportable under 10CFR21; however, we recognize that laboratory testing has its practical limits. |
| d. | Additional data supplied by Public Service Electric and Gas Company supports this conclusion. |
| e. | We have discussed our test results with the cognizant engineer at Public Service Electric and Gas Company and our concerns regarding the practical limitations of laboratory testing to duplicate a field environment. |
| f. | As a result of these discussions, NLI has accepted an invitation to attend the next scheduled start-up of the diesel generator. The purpose of the visit is threefold: |
| | 1) | Obtain firsthand knowledge of the conditions that precipitate the premature over voltage protection actuation. |
| | 2) | Provide input to plant personnel regarding the potential sources of the high levels of noise appearing on the DC output of the power supply prior to premature over voltage protection actuation. |
| | 3) | Obtain sufficient information about the power supply, the circuits for which the power supply is providing DC voltage, and related circuits around the power supply to finalize a conclusion regarding the reportability of the premature over voltage protection actuation in accordance with 10CFR21. |
| g. | The diesel generator start-up is scheduled for late August. The specific date has not been determined at this time. |
| h. | The requested extension date will provide sufficient time to document the results of the NLI laboratory testing and the results of the field-testing at Public Service Electric and Gas Company for inclusion in our correspondence with the licensees and the US Nuclear Regulatory Commission. |
